The Campbell County Health March 2019 Department Discovery features the
Care Management team, which is part of
Quality, Risk and Care Management Department in Gillette, Wyoming. Generally speaking, CCH's
Quality department monitors and evaluates trends within the organization, and
uses that information to improve our patients' experiences, increase
patient satisfaction and ensure patient safety.
For those who don’t know, Care Managers develop and coordinate continued
care services for those admitted to
Campbell County Memorial Hospital and
The Legacy Living and Rehabilitation Center, such as referrals to
home health care,
home medical resources for medical equipment,
hospice,
rehabilitation, IV home infusion, and other post-hospital resources. Care managers are
tasked in finding and referring patients to a multitude of community resources
such as transportation, housing, pharmacy and financial assistance. They
work with all kinds of patients, including those in dialysis,
oncology (cancer), the
ICU and
medical/surgical units,
maternal child (OB), the
emergency department, and the
clinics, among others.
The team is lean, with the following on staff: Crystal Peterson, RN, and
Jeanne Sturdevant, RN; along with social workers, Stephanie Berens, MSW and Dena Knox, BSW.
Amber Jackson-Jordan, RN, will also fill in when needed and help with taking call. The care
managers are available Monday-Friday and on call Saturday.
Care managers work hard to connect with the patient and often times the
family on a personal level to make sure the needs of the patient are met.
This helps CCH provide excellent patient care, and allows for more effective
management of their health conditions, which ultimately means fewer hospitalizations
for the patient.
